Sick gem tang

Recent history: tank went a few months without being serviced with a couple of weeks without filtration. The salinity level got down to 1.020. The gem tang wasn't very active, mostly hiding in the rocks. After servicing the tank, and raising the salinity back to 1.0235, 'she' was more active, not eating nori or lettuce, which she usually loves. Now she's getting these lesions shown in picture. What should we do?

Tank info: 125 gal, 6 ft long, 200 lb live rock, live sand.
Lighting: 250 W radium 20K, 96W SunPaq daylight 10000K. 96W SunPaq dual actinic 420/460. (I think these are right)
Corals: toadstool, ricordia, spaghetti leather, blastomussa
Fish: gem tang, one spot foxface rabbitfish, black ice clown, damselfish, blue green chromis, blennie

Try giving us more info.

When was the last fish/invert/rock added to the tank?
Why did the salinity go down and not up?
What do you feed her?
Aggressive tank mates?
Other water parameters, nitrates specifically?

From the spots on the tang it looks bacterial possibly or urenoma maybe? What was the last thing added and when? I would tell your husband to setup a QT tank, like a 40g breeder ASAP

HLLE? Head and Lateral Line Erosion? I've dealt with rehabbing a tang who'd had this problem, which seems to come from environment and diet. Try dosing food with SelCon. ANd feed greens, formula 2 and nori. It helped that one recover.
