Sick Naso Tang

I have a small Naso in a 20gallon long QT with a HOB, seeded sponge filter, and nano powerhead. He ate for 2 days after i introduced him and i treated the tank with 1.5 packets of General Cure (1 packet per 10gallon instructions). Eating dwindled. After 72 hours i did a 60% WC. Not much interest in swimming. He just sits at bottom and is breathing heavy but has no visual signs of disease. I have plenty of aeration. Any ideas on how i need to treat?

Temp 77.5
Ammonia 0.014 (done by UV/VIS chemistry test).

Why did you only treat with 1.5 packets of GC (which treats 15 gallons) when you have a 20 gal QT? Is the QT not filled up all the way or something??

Yes, only 15 gallons in the tank. He is looking worse as time goes on. LFS suggested i dose the tank with paraguard according to directions. I did that but no change. I tried a closeup of the fish and i see what could potentially look like white spots but not sure.

They will sometimes look splotchy when stressed. He could have parasites in the gills (like Velvet), which a freshwater dip would help with, but only copper or Chloroquine will 100% eradicate.

If he doesn't look better by tomorrow i will start chloroquine phosphate

Doesn't look like he will make it. this morning huge white blotch on both sides. i have no idea what this is or how to treat. Paraguard in QT did not help

within minutes of taking that picture he is dead. this hobby really sucks sometimes. would like to know what happened though.
