Treatment not working, things getting worse

Keep the tank as clean as possible. Just by looking at your HT I can see the film sick anemones make that hasn't been cleaned. Next water change use a never been used clean dish brush to clean all of that film off as that is making the water quality horrible and hopefully you will see an improvement. Good luck and you can't save all of them, just the nature of the beast.

That's what I've been doing. I upped it to 500mg and that seems to be when things got worse so I went back to 250.

Why are the nitrates so high? Are they that high in your DT?
Try half and half. Trates should be under 5 with a Mag... It can't get better with the constant stress of the nitrates...
If you can't get them down.... Honestly give the Mag to someone who has a more established tank without a nitrate issue... The animal will just slowly wither away without proper care and environment.
Just offering advice to save the animal... Mags are considered an "expert" care level animal for good reason

My tanks 6 years old nitrates are always that high all other tank inhabitants are fine. Frequent water changes never drop them lower. The nem is doing good now, inflated mouth closed and tentacles extended. I think it didn't like the 500mg of Cypro ever since dropping it back to 250 it's looked better.

Can you clarify what other tank inhabitants are doing fine with nitrates that high? H. mangnifica? s. gigantea/haddoni? acropora sp.? While hardier corals from more turbid environments such as some species of LPS and softies may do fine with higher nitrates, highly sensitive creatures such as those listed above will not tolerate it. IMO, magnificas and giganteas are the most demanding species in the hobby from a water quality perspective. I agree with lawdog that the nem is doomed in such water quality.

dd mags seem to have way slower response to cipro. i don't guess they treat mags with cipro but i guess they fed em
they tend to blow out tons of brown things and white strings
after you see a huge huge chunk of brown thing, it will go up again'
