Tube worm growing on clam & clam stopped opening

I have noticed recently that the clam is not open like it normally is. This tube worm(?) that is growing on it has gotten larger. It looks like it should still be able to open up.
How long can it go without opening? What can I do?
Unrelated question.......if a clam is already anchored to a rock, can it move if it wanted to? Also, please feel free to add any info about health and care, how they should look. I kind of inherited this clam. It's under 250w DE Phoenix 14K, anchored to the rock. Once in a while I have been dumping some Phyco Pure and/or Nannocholarpsis (sp?).

Thanks for the link. I went and checked as best I could and didn't find any snails.
It looks a bit better, but I was worried I was missing something with water quality so I did a 25% water change. Since its stuck into the rock, then would it impede the clam opening up? If they can't open all the way, would they move?

There are no fish in this tank.
Maybe I should get a wrasse to take care of any snails, nudis, etc?
Would any wrasse do? This is a 75 gallon tank.
 
Thats a vermetid snail. I asked before since I had one growing on my maxima, but most agreed that it wouldn't pose a problem. However the snail on mine died probably 2 weeks ago, but it never appeared to bother the clam much.
