Wall Hammer Tissue Recession

Ive had this hammer for about 2 years with no issues until now. One of its "arms" has no tissue on it now and it looks like the others are slowly dying. This coral was fine yesterday. There was a little brownish green slime where the tissue once was but i blew that off looking for live tissue. Anybody know what the heck is happening?

Hope someone chimes in, mine is doing the same thing and I've had it for 4 years. Water para's great, no phos or nitrates 1.026 cal 420 mg 1400 ph 8.0 8.1 alk 3.2 mg/l temp 78F I'm baffled to what is causing this because i just lost 2 heads of frog spawn to the exact same scenario. :headwally:
 
Is it near anything with sweeper tentacles or anything? I had a frogspawn that was destroyed by my war coral because I didn't think the tentacles could reach 5"...
