What happen to our clam???

We fear that it is too late to save him. We bought him and brought him into our reef tank about two weeks ago. He has appeared to be doing excellant. Look at picture below.

Then within one night (say 16 hours) he when to this. Huh?

The Nitrates, Phosphates, and pH levels are all healthy. One night; and now he looks terrible.
WHAT HAPPENED???

What fish and inverts do you have. List all of your water param's. What kind of additives do you use? How did you acclimate the clam?

i had the same thing happened to me with a nice squamosa, i had some type of a blenny ended up killing it ...
