What is this

Any ideas what this is? This fish has been in the tank since 2007, last fish was added Oct 2015. I did add some coral last month. This fish had the same think earlier in the year with I was adding phosphate remover. I stopped adding the remover because I and a few others thought the chemical could be the issue. Once I stopped the fish for better. Was 100% but now it's back. It came back after I added 2 Gyre pumps. Let me know your thoughts, I can not get it out to treat it because the tank is big and has to many rocks.It's still eating.

I can only tell it's covered in something.. not a very good picture... better lighting might help... more description of what you noticed for behavior, how long its's been since it started, how fast it spread, and what it looks like. Any other fish have it? What other fish do you have(I'm guessing a lot in a 375..)my first random guess would be ich from the coral that was added a month ago. (But the spots I don't think look right unless it's an optical illusion from the lighting)
 
The spots look like ich but larger and almost like skin.
The fish usually stays in the rocks so its hard to get a picture. The only difference in the fish is its staying near one cave now and comes out to eat.
As of now no other fish have signs of anything.
I would say he has been like this for a few weeks.

See If the "spots" are moving over time. To me it looks like it could be a really bad case of skin flukes.

I just find it odd that it went away for several months to only come back when I changed something in the tank. I will see if I can tell if they move.
Kinda looks like Brook hard to tell.

I would love to get it out and see about treating him but I have tried before and its a pain with that fish!
 
yeah only like food floating in the water. I was able to get some other fish in it but never the one I wanted.
 
How about fencing off a part of the tank with eggcrate where you can then catch it easier?

If nothing else works you can always try a barbless micro hook as the last resort.
