What killed my magnificent Fox face?

I had a weird situation happen recently, and I'd like to get peoples advice on what it was.

I recently got a magnificent fox face, and put it in quarantine. I notice its health deteriorate, and it started to get white spots on it and it's fins looked to rot a bit. It ended up dying (and I think it may of gotten other fish in QT sick too, but not sure), and I have no idea why. Take a look at the pic attached - you'll see the very apparent white spots. If I remember correctly, I think it was also starting to get dots on its face as well.

Anyone have any idea what it could of been/caused it? The water was at standard salinity, and I didnt treat with anything at the time. Are foxface's known to have some sensitivity to something? To add, the fish was very lathargic acting.

Really? I figured the white spots would mean something. The behavior was extremely lethargic. Not much else I can say. It also looked like the fin tips were rotting/turning white. It also ended up killing 2 other fish in the QT tank (copper band, mellanarus). Start to finish, maybe 3-6 days. I didnt see much else visually on it.
 
Both ich and velvet leave dead fish so diagnosis of a dead fish in a picture will not be useful. Ich is not a quick killer (eventually it may but not immediately) so velvet is the most likely (especially with the additional information supplied) with the time line supplied
 
Yes, I'm talking about the discoloration. The picture posted above is actually while the fish was alive, so yes they were before death, and got progressively more worse in size and color. Also snorvich - I'm pretty sure this was not a case of ich. I've had some experience with ich, and have an idea on how to diagnose it. In this case, it happened too fast, and also affected and killed other fish quickly as well, which leads me to think it wasn't ich.

As I said above, the mortality timeline is not consistent with ich. "Ich is not a quick killer (eventually it may but not immediately) so velvet is the most likely (especially with the additional information supplied) with the time line supplied"
