Probably a gall crab. If you tap on it, can you get it to move at all? If so, I would take a small pair of tweezers and try to pull it out.

if there is a crab in it remove it and dip it so is does not brown jelly up and receed.pluss feed minced foods with vitamins for healing.dont use large pieces of food unless minced they do not digest it.small meals over time is the best way to do it not one big meal here and there

WOW!! Ok... I just pulled the brain out and with some tweezers pulled the gall crab out!!

Sorry for the crappy pic.

nice job he should heal up great .i hope you dip it in some antibiotics so brown jelly doesnt accure.the crab would eat and keep the cut clean as he lived in it so it diddnt get infected and destroy his home so be carefull now that he is out of the whole so necrosis doesnt start.gl this should be stickied to help out future reefers with this prob.

Congrats on the extraction! Thanks for posting this as i have seen similar holes in brains at my LFS now and then. BTW, nothing like a slimy green brain (pretty one too) on a fresh white rug :).

While a dip isn't going to necessarily hurt, it is FAR from required. They can go through a LOT more stress than this and recover in literally days in a healthy tank. Dipping can sometimes do more harm than good.

dip is def advised you allways decontaminate cuts the crab can leave nasty bacteria from the shell alone in wich he keaps off of his host to keep his home safe i myself have been hospitalized from being poked by a crabshell and getting septice my arm was so swollen you could see every vein pulsing red.you make youre own choice and it was the right one to dip.if ciral is tough it will only help.i also like the slimy brain on the rug.dont show thw other half the pic.blaim the stain on a dog if you have one.lol i dont understand why someone would not dissenfect after removing a parasite from deep inside the body.just because they are tough does not mean they are invincable.i had a poerhead fall near a frogspawn and the high current made him drop a few tnetacles and one landed on a huge green brain i had.it looked good for a week then the infection started and it whent so fast it was all coralite in 24 hours.i bet he would have loved if i dipped him.
