whats wrong with my flame angel

I am fairly new to reef tanks and need some help i diagnosi g and treating my flame angel. He is still quite young and has been hiding alot lately. Also he will randomly swim to the top of the tank and make a splash then move really slowly.Also just today i noticed when he was swimming through some live rock caves he started vibrating. Visually i notice a large thin clear patch on his side and one small white mark behind gill amd fin on the same side. He is housed with one occelaris and a peppermint shrimp as well as some hermits and snIls and live rock.

How long has the clown been in the tank? Did he show any signs of any disease beforehand? How long have you had the angel? Are they eating?
