Zoas disappearing

Up until a week ago I had thriving zoa colonies and they are disappearing daily. Have no idea what's happening. Any ideas.
 
Same thing is happening to me as well.. My beloved utter chaos wont open up anymore. Only frags have melted in my tank as well as sps frags too. Found out that my phosphates were high due to the water i had been getting (was at 1 now at .25, The water i was using was between 2 and 5) But im still thinking its something else. Tomorrow im running a voltage test to see if there is any leaking (i allready unplugged my heater) grrrrrr What are your parameters? Have you checked for snails or nudis?
 
I have some zoas that hasn't been opening as of late. Had a closer look yesterday and there were some flatworms on them. I thought it was because they were still adapting to my tank.. now I'm trying to figure out how I should approach this :(

Though I highly suspect it is a fish or invert as both would, can and will clean polyps down to the rock overnight. We have seen this many times before. I will try to find some of those links for you.

Can you list your fish and inverts?

I don't think flat worms will eat zoas but they will irritate them. You could use flat worm exit to get rid of the flat worms but you will also lose a lot of inverts.
 
Also, did you notice any polyp retraction before they disappeared? If so, I would be highly suspicous of possible nudibranchs.
