2 in maxima dying

I had it for almost 3 months with no problems. Today, it just left it shell open and it looks near dead. There was no gaping or what so ever. What cause it to die like that ? Could it be because I did not target feed it ?
 
specs are

1.025
20 NO3
8 dkh
8.2 pH

I did not do anything to the tank other then normal dosing of kalwasser. The clam just looks like the muscle holding the shell got lose and it just left its shell open.
 
I got ATI T5 with individual reflectors. 1 Year old tank. temp 77-80.
0 ammonia.

Lifestock :

2 percula
1 diamond goby
1 six-line
1 scopas tang
1 dotted cardinal
1 blue-green chromis
1 purple firefish

2 other crocea that I got for 6 months
Turbos, crabs, one urchin.

Just another thought. Have you checked for pyramid snails. They feed on giant clams causing a slow decline. They are very small and easy to miss.
 
One thing that happened was that I left it at my LFS for near 2 weeks. It went there attached on a piece of rock. WHen I took back it was out of its rock and was on its side. I put it back in my tank and it gaped. It went back to normal after a day. And that was one and a half months ago. After it went out of its rock it never attached to any piece of rock I put it on. I had to epoxy it onto the rock on one side.

Most likly death from an injury with its byssal

Is it possible that its inability to attach anymore would probably showed the problem it has with its byssal ?
 
So how do we tell if a clam has injured byssal organ and what are their survival rates with injured byssal ?
