It's not old age, unless someone had him fifteen to twenty years before. A lfs near me has a 16 year old pair-male guards a nest for a week until the larvae hatch and get eaten. He gets about two days to rest, then does it all over again, and again, and again.

I would put her in a QT for a little while, to make sure she is eating..plus if she needs any meds it can be done without worrying about everything else in the tank.

Pictures can be deceiving, but the lateral line is quite prominent, the mouth appears open, and the belly appears plump(not what you would expect after a few day fast).

How long have you had the fish?
Ever laid eggs?
Any signs of discoloration around the mouth(red or white)?
As she sits in the corner, swimming in one place in a stiff manner?

have had for last 5yrs. never laid eggs, does swim in cornor, kinda stiff like, mouth looks fine, mouth is open alot, like kind of short of breath.

If you have added any coral, frags, live foods, other fish, etc., lately it could be a long list of things. Don't even want to guess prior to additional symptoms.

If you have not added anything of late and the only change has been removal of the sand bed, I would guess that may have stressed out the little lady.

Guesses:

IME, mature females seem to be more sensitive to ammonia spikes, O2 reduction, etc.

She maybe be egg bound or have other internal issues. Additional, and more clear pictures required.

Best of luck and you might consider moving her to quarantine, lowering the salinity, and getting ready for medicating as other symptoms appear.
