1. Initial Conditions (type/size of frag/coral, general health, AEFW infestation (yes/no), new addition or existing coral, any other useful info)
2. Medication Type/Brand
3. Medication Concentration/Dosage (ppm, number of tabs, number of drops, etc..)
4. Dip Duration (20 min, 6 hours, etc)
5. Total Dip Volume (1 gallon, 5 gallons, etc)
6. Dip Environmental Conditions (water motion, water temperature, or any other conditions)
7. Number of Dips
8. Quarantine Period (none, 2 weeks, 4 weeks, etc)
9. Quarantine Environmental Conditions (water motion, water temperature, lighting, filtration, skimming, tank size, live rock, live sand, bare tank,etc.)
9. Initial Results
10. Long Term Results
11. Other Comments, Observations, and Conclusions
If you have no data on one of the categories just state that is the case and complete as much of the others as you can.
