Anyone know what's wrong with this leopard wrasse?

Is the tank cycled?

It was completely cycled, yes.

The only thing new was
0) added rinsed sand
1) added pp
2) removed carbon
3) removed poly filter

Not in any particular order, but the tank was completely cycled beforehand.

I'm so frustrated with disease!!!!!!!!!!!!!

I had a bad ammonia spike and bacterial bloom!!!!

I think it's ammonia toxicity! I asked my wife to feed them if she sees them pop out, but she hasn't over fed them!

I tested the water a couple of days ago and the ammonia was nil! What the heck?

How big is your tank and what other fish reside in it? Im seeing three leopard wrasses and if all added at once depending on the size/age/Bio-load of the tank adding 3 fish at once could cause a bacterial bloom.

Cant say for sure without knowing more about your tank but its a common issue we had at a fishstore i used to work at. some Tanks would be cloudy on delivery days and a day or two after due to the sudden addition of so many fish to the system. I usually add a bit of Bottled Bacteria when adding more than one fish to an established tank.

How long were the carbon and poly filter there prior to their removal? You may have removed the majority of the biological filtration.

Crap!!!! You're right evolved!!! They were in for a long time!!! Oh no, you're correct, but I have a sponge that's been in just as long that I left and the sand bed has been in for a 2-3 weeks. Omg. Didn't think of those two things!!!

I love bio spira. Good stuff. Better than microbacter.

Thanks for the tips. I added poly filter back in, did two massive (3 gallon!) water changes and the wrasses (except one) have found their way back into the sand- good sign eh?

Added an extra air stone just in case the bacterial bloom killed the Oxygen in there.

I can't control the bacterial bloom! I dose prime each day and it's milky still. Do water changes and no effect.

The wrasses are alive, but not looking well. One is alive, swimming upside down and has very alert eyes and fin movements. Another one is lying on its side but responsive to light. The other is buried in the sand with mouth protruding out. They're hypoxic, so I know to run an extra air stone.

Alive, but im sad looking at them. I can't help but to think i inadvertently did this to them by removing some media. And boom, ammonia spike (may be false with prime and ammo lock?) and bacterial bloom.
