Clown getting beat up. By who?

I'm not sure what is attacking my ocellaris.

A few weeks ago something took bites out of its top fin. It healed.
Today it is now missing its left pectoral.
It has been bitten so short, that I don't know if it will grow back.

The clown is in a 40 gallon breeder along with another clown, a golden sailfin blenny, royal gramma, pygmy angel, and lantern toby puffer.

I don't know which one is the culprit.
I'm guessing the puffer.

Any thoughts?

I'm thinking I will take the toby puffer out.
I've been meaning to anyway to reduce bioload.

Any help would be appreciated.
 
In a 40 gallon tank the Angel may also get aggressive, but its aggression would most likely be notable on a regular basis, not a hit and run nip.
 
Yes both clowns are ocellaris.
And they have been together over a year.
They have a adopted a hammer coral.
 
The tank is at my work office.
I don't usually see aggression while I am at work.
I only see the Royal Gramma flare up once in a while.

But its happening while im not here.
