Dying Condylactis?

I have a blue-tipped condylactis that is in serious distress. The tentacles are mostly translucent and have been reducing in length. Water parameters are good but a few days ago there was a salt spike from 1.025 up to 1.027. Not sure how that happened. The nitates are also about 8. I wasn't sure if it is a "no hope" situation, or if there is something I can do. I also didn't know if I should try to feed it, or if that would further stress it out?
 
How long have you had it?
How old is the setup it's in?
How much light is there?
What size setup is it?
What else is in the tank?

Without having any real info to go off of, shortening tentacles means it is starving, either for light or for food. Translucent tentacles usually means that it is unable to process light and requires more feedings than a healthy anemone.
