false perc sick

I have a false perc I've been watching for a while in the tank. She didn't seem to be eating for a while. But didn't think much of it because she would stay at a normal weight. She would only swim in a corner of the tank with the least amount of flow kind of on her side. Recently I've noticed that her gills seemed swollen and mouth was always open. So I went fishing and she didn't struggle as much as I thought when getting her out. I have her in QT and she really took a turn for the worse. I know now that she dosn't eat at all and has become very skinney. Gills seem more swollen than ever and the white stripes look pink. I've been doing 50% water changes once every week and am running carbon to no avail. Her partner in the display tank is showing no signs of the same thing and neither are any of the other fish.
I will put her down tues if no one has any ideas of what it is or can help. So any ideas? I hate to see this fish pass. I've had her for about 2 years.

Do you have a picture? A picture would really help.

Do her gills show any abnormalities, look funny, see any discolored spots or anything look out of place?

How long has she been in the QT?

Her strips look pink; meaning it looks red and blotchy?

could be worms, could be a bacterial infection that is systemic or ?
 
I'll get a pic up as soon as I can. Batt. just went dead in the camera. I'm not so good at taking pics and the camera isn't the best but I'll get somthing up as soon as I can.

Her stripes seem to have a pink haze to them. The very outside of the strip looks to still have a little color. I was looking close to night and around the gills apear to be red all the way back to the fins. So yea they look abnormal. Looks like there may be a white spot on the gills. May be a piece of food I put in yesterday though. She's been in QT for about 3 weeks.
