Flame Angel Almost Dead

Very discouraged...

My flame angel is in QT now. He's almost dead. I've had him almost 3 months. He spent 1 month in QT. The last week I noticed him not pecking much at the rock, and within the last 3 days he was swimming very weakly, as if there was too much flow. I moved him to QT today, as a last resort because I thought the stress would be worse for him. I googled about it and found that it looks a lot like swim bladder problems.

He was by far my favorite fish. Just very discouraged as I've lost a purple tang, foxface, and now flame angel in the past 5 months.
 
Those are all fairly tough fish.
Something in your tank is not right, you need to find out what the root cause of the deaths is.
 
Definitely this is not normal. You could have a hidden killer, like a mantis; you could have flukes, or an oxygenation issue. Tell us your total tank data: parameters, equipment, size, etc.

All of the water surface is moving..

Seems like all fish that died had a bulge in their stomach.. the flame is still barely alive in QT...haven't seen much of him today as he's hiding behind rock... I'm treating with Maracyn 2 in QT.

Tank has been set up a year..

Fish:
Purple Dottyback
4" 2 bar rabbitfish
Orange Spotted Goby
2 Oscellaris Clowns

Inverts:
1 cleaner shrimp
1 peppermint shrimp
1 pistol shrimp

The clowns have been in the tank almost a year, goby almost 8 months, and dottyback about 9 months... all with no issues...

It's really tough to see a fish suffer...
