Yes, they all start like that and perk up when you put them in clean water. Keep a vigilant eye for deflation and start treatment if it does.

I would treat right away. Looking at the photo, that's a clear sign of infection.
 
Bio filters pulIled, all water changed, lights out....250mg cipro...here we go! I cant turn on the PH because he will blow all over the place...Does anyone see this as a problem?
 
I would completely divided the HT to two compartment where the anemone cannot get from one side to the other. All you need to do is to cut the egg crate to appropriate size and use some flexible air tubing (or something similar) to jammed it in place. Then you don't have to worry about the anemone get into the heater, or pull into the PH. I would point the PH to the surgace or the side, this would change the flow to just right (or else use a Vortech PH)
 
Was able to section off one side, got the PH goin, and hasn't blown him off the cup yet. Still looking like death. Deflated, mouth open...yay!
 
position the cup, especially the handle to squash him against the wall of the aquarium. He will attach after a few hrs. You really need to get the circulation going. That is why you should separate the anemone from the equipments.
 
It's a game of patience now. It is imperative that you keep the water as clean as possible.
When you do water changes, best is to clean the tank walls and if you have spares, change the power head with a clean one and wash the one you just pulled out for the next water change.
Normally you will see a positive reaction after the second day (or earlier), however, if it stays pancaked all the time it might sadly not make it.
 
It is back inflated now. Took about 3 hours after cipro. Hopefully it stays that way. Will do another water change and cipro tomorrow at lights out. Will update with pics in the a.m.
 
By the way, srry for the crappy phone pics, its the best I got. In the first pic you can see a brown/black poo at about 11 oclock. In the second pics its just above the nem. He is still expelling it so hoping for the best.
 




Lights on tried to get the zooanthella being expelled best I could. Just actinics on but it looks like the water is a little cloudy. Cant tell yet. If it is cloudy, would I change water and add more cipro or just change out half water and do the full change out this evening?
 
Taking out the water will reduce the cipro.

I have replaced the water 100% twice in a day before, as it got to cloudy for my desire.

The cipro will degrade with the light so I would wait to readd it at next lights out, if you decide to replace some/all of the water.
 
When you change the water add cipro. Does not mater day or night. Cipro does get degrade with light but add it anyway.
If the water is clowdy I change 100% of the water and dose 250 mg cipro. Then I change again and dose again at light out
 
It was just the actinic lighting I think. Halides are on now and water is clear. Nem still looks really good. Mouth is still open about 1/8". So seems to be doing good. Expelled a string of goo and dead zooanthella about 2 inches long.
