Help identify sore

I had a different account but I lost the access to that one.

I also started treating with levofloxacin and metronidazole to cover all bases.

He ate a little bit of spectrum hex shield pellets this morning. I have not attempted feeding this evening as I need to dim the lights for the meds.

It started as a little bump a few days ago around the time when he freaked out and hit the tank from inside. I don't know where he hit and it progressed to a redness seen in the photo. The tank was having cloudy water issues and I just moved it to a quarantine. I gave it a freshwater bath while I was filling the quarantine.

Could uronema have survived 3 years? I used formalin dips between tank transfers and freshwater dips. Also treated with prazi (3 rounds) and chloroquine briefly as a preventative. The tanks were bleached and dried in between transfers.

I think the most likely scenario is that it injured itself and bacteria started attacking. Perhaps uronema proliferated due to the cloudy water (organics) and attacked when he injured himself?

He is behaving as if it doesn't bother him.

Just an update.

He hasn't really gotten worse. Maybe slight healing.

Based on pictures of vibrio and the rate at which it occurs, I am quite sure it is not vibrio.

My treatment with metro has been discontinued and again, based on the rate of occurrence, its not uronema.

Currently continuing levofloxacin as prophylaxis.

There is a center white bump that is raised.
Based on the current state, I assume that mycobacteria or lympho is most likely. The white area seems to be slowly shrinking.

Furthermore, I strongly believe that it is not mycobacteria, since mycobacteria sores usually occur in the latter stage with additional symptoms of skinniness and lethargy. The fish is acting completely normal and not skinny.

We shall continue and see.

It seems like the bump was grown from within the fish. When it grew it ripped a hole. Around the white bump is inflammation, which cause the red appearance.

This further suggest that lympho is the likely candidate. I've seen lympho on the gills of large angels before as well.
