is this hole in foot disease?

my lta which I purchased last month was very healthy and happy and firmly attached to the sandbed, until last week.
it just floats around here and there and doesn't want to attach.
I even tried burying its foot in the sand, but it just got out and started floating around.
my maroon clowns are still hosting in it.
the tentacles are very sticky and the mouth is fully closed.

My GBTA that just died this past weekend had a hole through also. I thought it was trying to split when the Clown fish got to him. I had him isolated for a little bit and I thought he was going to recover but he eventually gave in. Did you notice it trying to split? What was he doing right before he started floating around?
 
it was burried in the sand, and the clowns just started hosting in it
then the next day I see it has moved to behind the rocks in the corner of the tank.
Next day, I saw it stuck on a powerhead, luckily it was just for a min or two.
thats when it stopped attaching itself anywhere.
the clowns still host in it, infact the clowns never used to move from them spot, but now they'll go searching for the anemone when moves around.
as you can see in the pics, theres no intestine or anything thats coming out of the foot, but it looks like someone took a needle and pierced right into the foot.
Maybe this is just the begining. I hope now.
hopefully it will find a nice place to settle in

By the looks of the picture, the column and foot do not appear damaged at all. What is your lighting and water parameters. Also you may want to put him in a low flow area of the tank and see if he attaches his foot.
 
I cut down my flow by half so that he hoesnt float around, but its still the same now after a week. he just refuses to attach.
my parameters are
ca - 520 (api)
alk - 7.5 (goes down to 7 sometimes) (salifert)
po4 - 0.02 (seachem)
no3 - 2 (salifert)
mg - 1280 (sera)
temp about 26C
lights - 2x39w 460nm blue, 1x39w 420nm actnic, 1x39w 10k white
2x150w mh
tank is a 75g with dims as 100cmx50cmx60cm (60 being height)
 
I know it isn't an LTA, but I had a GBTA that cloned and the clone would not attach. He just floated around so I put him in the corner where the flow was the lowest and waited. After about a month he finally decided to attach and then moved to a spot he likes. Since, he has not moved at all and is happy as can be.

I really have not had any experience with hole in the foot, so I cannot say anything about that, otherwise your anemone looks very healthy. I hope this gives you a little hope, and good luck.
 
Don't give up. Mine took awhile to get settled in, and moved around a lot, including rolling around on top of the sand. I agree that the foot looks ok. Can you remove the clowns temporarily to see if the anemone would settle down?
 
The pedal disk doesn't appear to have a hole. When there it a hole, it's the result of the anemone being ripped from something it is attached to. It's not a disease per se. However, with a tear there is the risk of an infection developing, especially in that species. I would suspect either water quality issues, flow, or lighting if the anemone isn't digging into the the sand bed and attaching. A deep sand bed is needed for LTAs--5 or 6 inches of fine sand, for instance.
 
It doesnt look to have a hole in the foot... As said above I belive it just needs time to settle. I recently upgraded my tank and it took my LTA 4 weeks to attach I guess it will stay there it has been attached for several days now so I have my fingers crossed!
Just wait for it to settle and keep params in check good light and moderate to low flow areas where it can settle.
