Juv. Emperor has two small holes on it...(see pic)

I have no clue what's causing these two small holes on my juv. emperor angel (you could see right through it). The holes started out as bumps at first, then decreased in size until it became a hole. He's eating and swimming okay. Anyone have any idea? I circled it in red...

It's dominant in the tank, but not the biggest. Hippo Tangs, Singapore Angel and Domino don't mess w/ it. LFS thought it may be a wound from another fish, too. But I think it's something else. Not pictured is another bump that may or may not end up as a hole like the other two. I'm adding Selcon to their food and dosing Vita-Chem a couple of times a week in hopes of boosting their immune system. Tank is only about 3-4 months old.
 
Update: Unfortunately, the two small holes seems like they're becoming one oval-shaped hole and seems to be getting bigger. Plus, it has a similar bump near the hole that may make things even worse for the lil guy. Other than Selcon and Vita-Chem for supplement boost, I don't know what else to do. None of the other fish have this, all are swimming and eating as usual. Also, water parameters have been tested and are fine.

I'll post another pic if it gets much worse.
