Test you display tank for Amm/nitrite/nitrate....You added an antibiotic to the display...it may have affected the zooxanthella and your nitrification cycle. Corals area stressed and tank could be re-cycling.

Found him dead in the HT tonight.

He was turning around and eating very aggressively. I thought he was going to make it. Last night he didn't eat though. Today he was dead. The HT never really seemed to cycle. Don't know if it was the medication or what. I was doing 50% water changes a night to keep the ammonia in check and dosing Prime. I thought maybe the ammonia was a little higher than normal yesterday but I didn't test. Just did the usual water change.

For the record I started treating with Prazi in the HT. I did that for a week and a half or so. He seemed to get worse so I switched to Metro. This seemed to be helping and was eating aggressively but still not putting on weight.

Pretty bummed out. Not only did I lose the fish but I lost a lot of nice coral trying to originally treat him in the DT.

Lesson learned.
 
Prazi will not work for internal parasites, but works great for flukes.
I had a fish that was suffering from flukes, and it looked a lot like yours, a pinched back and cloudy eyes, and eating a lot. Then it started getting skinner and got discolored spots on its sides and the fins started rotting away. I did a FW dip(2 min) and over 10 flukes came off my 2" wrasse. By the time I caught this fish it only lasted a few days in the QT. The disease was too far along.
IMO start treating ASAP with prazi. Good care, proper nutrition, and a properly functioning immunity system is not enough to fend off flukes, it requires medication. Good Luck.

I lost several acro frags. I lost most of a nice tabling milli. My digtatas have yet to fully recover. Some dead areas growing on them and very poor polyp extension. My orange cap has come back though. My green one is looking better but still has some algae growing on it.

I tried both prazi and metro at different times and lost him anyway.
