pink margin wrasse aggression

my pair pink margins chased and pecked at my tri-color until he developed ich and died. is this normal behavior?
i had them in my 40 breeder and the tricolor was introduced last.
now i'm scared to add any more in fare they will do the same.
how many fairies do you guys have in your system and what size system? thanks!

Your tank is too small for a fairy wrasse especially if you add another wrasse.
