So he's at least 10 yrs old...
It is possible that happen.Last fish was a Yellow Tang about 2 to 3 months ago.That fish was put in my quarntine tank for a month with no signs of disease.

Based on that I would assume that he has come down with something. I would agree that in this case it is best to pull the fish for treatment.
The only question is against what?
The first step after getting him into a QT I would do is to lower the salinity to a specific weight around 1.018 kg/l. This should reduce his energy consumption and increase oxygen saturation of the water.
What did you add to the tank over the last 6 months?
Did you use anything possibly harmful to fish (paint, cleaner, solvents, pesticides, hairspray, or other aerosolized products) in or around the house?
Do you see anything on the other fish?
Does he spread his gills open?
If possible take a video and post it.
Also it's probably best to start a new thread about this issue in the disease forums as you want to have more disease specialists looking into this issue.
id be more inclined to think that the yellow tang you introduced had flukes which angels, regals in particular, do not do well with. flukes are the ich/tang equivilent to angels.

That's why it is so important to give them a formalin bath before putting them into the QT.
I skipped that with a tiny regal because I was afraid it might harm him, but what killed him were the parasites in his gills - probably flukes.

I'd like to get some opinions here. It's about time to try another Regal and I want to implement the best strategy for survival.
I have only one source for such a fish and that is mail order. I'm of the opinion that buying from LA or LA DD is the best option. If I can get a yellow belly from DD, I'll pay an arm and a leg for it, but it has a much higher chance for survival. If I buy it from LA, I am going to have to go through a more aggressive and thorough quarantine process. Either way I get a two week guarantee which is nice, but the emotional toll from such a loss is NOT GOOD.
In preparation for this purchase, I have set up a forty breeder with live rock and all the trimmings for my quarantine tank. Is been running fallow for over 75 days now to kill the ich that was introduced by a pyramid butterfly. It is full of pods and has patches of hair algae on the glass. It's the perfect environment for a new regal.
Here is my dilemma. If I introduce a new regal to this tank that has not been preconditioned, like something from DD, I risk introducing ich back into the tank and having to go through another fallow period. I KNOW no DD fish precludes the introduction of ich, but the odds of the fish having it are a lot lower. If I buy it from LA, I don't want to introduce it into the breeder until it has gone through TTM or some other treatment. The thing is, is how to do it and come out successful.
If I put it into a bare bottom 20 long for observation, its transition would be more stressful and it could starve unless I introduce some small rocks for it to pick on. If it does come down with something, treating him in the 20L would be much easier...
What's the best way?

Unless you're going to have some other acquisitions soon that you need the cycled QT for I'd take the chance and put him in there. Much better chance of success. If he get's ich you can hopefully put him through TTM in better shape and already eating.

From the reports here I think NY Aquatics may actually be the better source for Red Sea Regals than LA.
With a regal I wouldn't worry too much about ich. My two got over it all by themselves. Brook, velvet and flukes are more of a immediate concern.
Also, I found regals like caulerpa and other macro algae, though I've never seen them touch hairy algae.
