never seen mine to be inflated like that, but on the other side - when mine was burring in 3" sandbed, it inflated his foot quite a lot during that procedure.
If u have less than 1" sand it could in theory inflate to some strange shape.
u still not provided water parameters. how deep is that sand? he need 2" at least for start.

Hard to say for sure, yeah, it is struggling a bit, hopefully just acclimating, but at least you have it in a better location.
Hopefully you will see it set foot, that would be a good sign, and then mouth begin to close, hopefully.
Be patient, but keep an eye.
