Sick Yellow Pyramid-help please

I have managed to get him into a QT tank and catching him and transfer were pretty stressful. Here he is right after capture and transfer



He is doing better now - 45 minutes later. It appears to be Popeye and maybe something else, or the Popeye is a result of something else. His dorsal fin stays extended.



He is slowly swimming in circles.







I have begun treatment with Melafix. Is there anything else I should be doing? Is Melafix a broad spectrum antibiotic?
 
IME Melafix doesn't do anything except make your water smell like tea. I would pick up a broad-spectrum antibiotic to treat the infection. I personally like SeaChem Kanaplex (kanamycin).

1. How old is this aquarium? The aquarium is a 180 and set up 1 year ago

2. If less than six months old, what is ammonia level?

3. What is SG of this aquarium? How measured? Salinity 34 sg 1.026 measured wit a refractometer, calibrated with 35 calibration fluid.

4. When was the last fish added to this aquarium? Last fish added 9/23/13, this guy was added 7/22

5. Was it quarantined? If so, how? And how long? Was it prophylactically treated? How? All fish were QTed for 6 weeks and prophylactically treated with Cupramine

6. If you are using a copper based medication, which one? How often do you measure level? When?

7. If you are using hyposalinity, how did you calibrate your refractometer? N/A

8. Please describe in detail, the appearance of the fish? See photos
If there is one or more pimples, are they lumpy? What color?

9. Please describe the behavior of the fish as best you can. Is it acting reclusive? Is it always up towards the top of the aquarium? Is it avoiding light? How active is the fish?
In the DT before transfer, he was staying up near the top in a corner swimming very little. He is not straight up and down but on a bit of an angle

10. Is the fish eating? What? When food was introduced last night he went to the center of the tank to eat, but didn't appear to get much. That was frozen homemade food. I also always feed nori on a clip as I have many herbivores. When the Nori was introed last night he did not join the others.

Other tank mates are Flame angel, PBT, Midas Blenny, Spot Breasted Angel, Bluehead Wrasse, Yellow Tang, Kole Tang, Purple Dottyback, Cardinal Dottyback, Bi Color.
 
Not to be gloomy Gus, but I have had recent issues with pyramids. Once they start swimming funny nothing I tried (furan 2, NGP) worked and the fish died.
 
It is with deep regret that we announce the passing of Yellow P. He passed peacefully in his sleep last night. Friends and loved ones were close by in the house where he had been maintained for the last 5 months. After a difficult last few days he passed of unknown causes and will be missed. He is wished happy swimming.
