SPS/acropora tissue necrosis?

So as the title states i am having an issue with either bleaching or tissue necrosis on a single frag. Im guessing tissue necrosis because its not something that is "transitioning" to that point its more like the skin is slowly peeling off.
I wish i could be more specific on exactly what species of acropora this is but i got it in a limited edition DFS liveaquaria frag pack, so im unsure.
When i first got it, it was completely browned out and looked sad. I acclimated it and its been in my tank for a few months and was doing well. it started to color up with fluorescent green polyps, and purple tissue around the tips slowly moving down its "arms" i was ecstatic. But a few weeks to a month ago i noticed it had a couple white spots in between polyps. Now currently it has about 1/4 of its body as just a pure calcium skeleton and no tissue.
Im almost wondering if this is a lighting issue, because the only parts of the frag showing this tissue necrosis are the shaded areas. My LED's get blocked out a bit on the body of the frag by its own branches.
I definitely think this is the slow tissue necrosis not rapid, but it just wont seem to stop.
My parameters are as follows:
79.9F
Calc: 460
Mg: unknown as i dont own a test kit but use reef crystals for a 5-10% water change weekly.
Alk: 9dKH
Phos: Undetectable with API
Nitrate: Undetectable with API
SG 1.025
Ammonia 0
Nitrite 0
Not sure what to do at this point. Any suggestions would be great.

On a side note, i ordered a hyacinth birdsnest from liveaquaria about 3 weeks ago and the thing is literally growing like a weed. I went out of town for 4 days total and when i came back it had over 12 new small "branches" that i could count. So i just cant imagine this is a parameter issue. All of my other LPS and SPS seem to be doing great and growing rapidly.
 
Post a picture if you can. A clear and up-close as possible one along with a full coral shot.

"couple white spots between polyps" has me worried. Are they almost perfectly round and small white spots? If so dip your corals because it's possible you may have AEFW (Acro Eating Flat Worms)

With your Phos/Nitates being essentially 0 or very little, I'd reduce your Alkalinity to around 7.5-8. Also, depending on your lighting intensity, this could vary as well.
 
Some acros make it and some dont.
Dont feel bad if you lose a couple of frags.
Once Acros go downhill there's not much turning back--frag it.

Or red bugs. I swear every red bug infestation I have had has come from a tri-color. To be fair, I have only had two instances, but still.

I would isolate and dip.

So after turning my powerheads off and doing close inspections of my tank im noticing another acro frag not doing so hot. One of my fish just broke it off of the rock and it fell and looking at the back underside it is not happy. Ill post a pic, im going to just take it out of the tank and snap a pic since i have to re-glue it anyway.
