Sudden death of PBT, looked healthy day before

I had this tang in my 125 for about 4 months. Eat pellets, seaweed and brine. I never noticed that he had any issues except some occasional ich spots. Eventually, he fights them off so I kept a daily dose of the nitrobacter-5 snake oil because I had some around.

Yesterday, he was doing just fine. No darting and hiding and coming up for food. This morning, I noticed that his skin look like he has been lashing himself at the rocks. He had these white/blue spots on his skin. Four hour later, he got worse and he stopped swimming. I was too late to prepare at QT for him and he died when I netted him out. So within 12 or so hours, he went from healthy eating to his death. I cant think of anything other than velvet, but i have never seen or experience this before. Also, I checked his gills and did not see any white spots in there either. The only other fish in there is a YWG and clown. Any thoughts?
