Swim Bladder Disease or Constipated?

My Kole Tang has had a small swollen belly for a month now and just the other day I noticed that it was 3 times the size. I thought it was constipated but would see it have a pretty large green movement when it would get excited for food. I am having a pickle trying to catch 'Elton' too. =( I have had him for 2 years now and this sux. Any help is greatly appreciated. I know the pix aren't very good, but that is all I can do with my camera. He won't socalize now and is hiding in a back cave in my rockwork. It is a little smaller than a ping pong ball sized stomach. :thumbdown

It hasn't been swimming any stranger than normal. It has always had a tiny lump there but this is 20x the size and sticking out both sides. I added some ricordea the other day and an acan died for some reason two days ago, but the acan never looked well for the month that I put it in there.
 
Now there is a small hole and a white spot where the swollen side was. I am baffled here. It is not nearly as large now.
 
Still swimming normally, too. Comes up with the other fish, as if nothing is wrong, but it had to have expelled whatever from there. Anyone have a clue? I will try to get another picture now.
 
Just a guess- sounds like some sort of internal parasite that somehow hatched out of your fish? Maybe treat with an antibiotic for the hole and something additional for internal parasites. I would keep an eye on your other fish.
 
I have about 10 fire shrimp in it and I was wondering if that would help with whatever it is that is going on, if it is parasitic. I have been in SW for 3 years now and this has me all sorts of sad.
