What happened?

Ok so I just recently removed my clam from the shell he was attached too (it was out growing the shell) and I placed him on a plug. He attached to that right off the bat and was fine untill today. Today I came home and found him on the sand (he was high up on the rock) and his foot was torn off his shell and was still attached to the plug. I set him upright onto the plug again but I am so afraid he is going to die. Right now he is open slightly and retracks to light but I dont know what happend. Any help is greatly appreciated.

bump* someone has to know what is going on here? or at least have some idea.... i just dont know where to begin on what to do for him.
 
I'm not an expert on clams, but I can state that my clam has lost its byssal threads before and survived. The clam had fallen over and my purple tang ripped 'em off. Might you have any fishes which could do similarly?

If the clam is still reactive, try to find it a new spot. It sounds as though it may have rejected the location on the plug after a period of time. Clams will move a fair amount until they are content with their location. My two maximas were irritatingly restless until I positioned them on some rock parallel with the lights and they've been there for 6 plus months contentedly.
