Who could puncture fish

Hello everybody,
could you help us with identification of death reason, please...
There is a fish from my friend's tank. It lived there for more than two years and yesterday it's died.
There was very strange hole on the body. It was found the evening before. There aren't definately any mantiss, crabs ets in the tank.
Other fish are surgeon, blenny, clowns. One lismata shrimp and some other inhabitants - all aren't able to make such hole.
It looks like parasite attack - but what exactly if so? - the last time something new (few Live rocks) was added into the tank - April and nothing like that had happened until this time.
It's very important for us to identify the reason and to avoid the continuation...

In a 100 liter tank, that fish was unlikely to survive.
This tank is not 100 l - why? :) - as I've written in the beginning it's a tank of my friend with surgeons etc... it would be strange to have surgeons in 100 l more than Synchiropus splendidus..
Oh, I see, you've looked at my profile I didn't update for a long time - and you didn't read my first post... Sorry. Updated. But - this is not my tank.

Anyway the hole has to have a concrete reason - and we try to find it to avoid repetition and will be very pleased for help with identification :)

But my friend definately didn't try to catch her somehow - as I've written at the first post this fish was more than two years in the tank.

I missed the two years part :headwally:

I really don't know what could have made such a small round hole. But some sting (copepod, coral,...) and a secondary bacterial infection would be my best guess.

In a 100 liter tank, that fish was unlikely to survive.

I have a pair now for a year in a 25 gallon (~100 liter) tank and they do fairly well. It is for sure doable, even if not recommended.

We thought about corals ets wich can bite the fish in the beginning. Nothing. My friend has Echinometra in her tank - but it lives highly on tank walls usually.. if only at night and accidentally..
Or - is there any possibility that this desease (infection, parasite) came "from inside"" of the fish and made this hole... Nothing certain come to mind.

About volume for this fish - agree with you :). I guess volume, sufficient for her healthy life depends on some points - size (age) of Synchiropus splendidus - they can be very different, way and habit of feeding, quantity and quality of live rocks, macroplankton population, presence of food competitors - such as Wrasses , for example, etc. It seems to me volume in itself isn't the most important parameter for this fish if she has enough food and enough place to explore the rocks...
