1/30/12 Update:

Found out that one of my T5 bulbs isnt working. It is the front one. Not sure if it is burnt out or what. Also the Icecap Deepwater 14k bulb is yellowish now. Kinda strange. It is the last bulb, and I am wondering if there is any correlation between those two.
 
1/31/12 Update:
Took some pics with my real camera (still a P&S until I upgrade to a DSLR)
but they are better than what you have had to deal with so far!

There are a bunch of plastic connectors in that fixture, along with screws that hold each bulb in place. Make sure they are all in place. Those non-wavepoint bulbs are not old. I got them at age of aquariums, maybe they can help you out. If a ballast is failing, the fixture came from Neptune's Reef in Torrance.

I will definitely check all the connections. I dont think it could be both bulbs at the same time, so I am leaning towards wiring or ballast.

2/3/12 Update:
Powder Blue Tang died today. I have no idea why everything is dying. Individually, the fish deaths makes sense, but this many this often is really concerning me. All test levels came out fine. Nitrate was about 10 though?

Some of the Lepidozygus Tapinosoma look diseased now too! Eye issues and dull coloration
 
I hate this hobby. All of my fish are dying and I don't know why.
Leopard wrasse died today. Diamond goby died today. I watched a Chromis go from free swimming with the others and looking completely fine to being dead in about 10 seconds. So I am starting to think this thing may be a neurological problem.

Crappy last week or two for me. And it doesn't look like it is slowing down

Really sorry to hear about that bro'... never fun when this stuff happens.

So you said your levels were all in check? Are any corals showing stress? Did you quarantine any of the fish before putting them in the tank?

One thing that may have made it worse is the addition of all of the fish somewhat quickly, but of course hindsight is always 20-20. The reason I say this is because if a couple of weeks is left to cushion each new addition, especially if you're not quarantining, then any new malady will typically show itself before the addition of any new fish and you can deal with the problem before the new fish arrive and possibly become infected themselves or infect the tank with whatever they're carrying along (thus compounding the problem).

Again, really sorry to see this happen. Hope your tank recovers soon. :(

No more fish loss. Maybe things are stabilizing? Maybe...


Added a nassarius snail and a hermit I found in the bottom of a bucket with LR that I was letting dry. He was in a small drop of water, dont know how he is alive after a week or so like that, no heater, no skimmer, no circulation. huge temp fluctuations, and I am sure a lot of nitrite.
 
2/12/12 Update:

Still no additional deaths! Things are really looking a lot healthier. There is almost no algae on the back wall now, and it is starting to thin in other areas.
I took the bright green carpet to the LFS because I couldnt keep up with it moving around and getting caught in Koralias. Algae Turf Scrubber is kicking in a little more. I still need to improve the lighting to it, and maybe redesign it so it is quieter.

I dont think I am going to do anything else to this tank for a while. Except regular maintenance and water changes, and see where that takes me. I want this algae gone, and a lot longer of a period of time to go by before adding any more fish, but with coral, who knows haha
 
After seeing your tank yesterday, I can say it looks 180* different from how it did when the ATS was first starting up.

The back wall of your tank is spotless now, and not from elbow grease, which is impressive.
 
Zero elbow grease, that requires too much effort :)

Thanks man, it has been a long time in the coming to get rid of that back wall algae. Glad to see it finally gone, hopefully I can keep it that way
 
2/13/12 Update:
Survivors are surviving, starting to look better and better after losing some coloration.
I took the acrylic sheets off of my ATS and it is running SOOO much better! The acrylic helped the water to escape from the canvas and made dry spots where it made contact, so now the whole thing is covered in flowing water, and it is MUCH quieter.. we will see how it goes. Still need to add another light on it, as I am not seeing the growth that I should.
