What you bought there is mutilated yellow tang, Zebrasoma flavescens, that someone has deliberately cut up its dorsal fin, in order to get that 'horned' appearance, and presumably a higher price tag.
I would really suggest going back to where you got it from, and confront them about it. It's a little appalling that they would be selling it as such, they really should know there's no such thing as a yellow horned tang. On the off chance they don't happen to know, have them contact their distributors. That's atrocious.

His dorsal fin is missing little piece and perfectly heal.Is part of his dorsal fin missing or is this a deformity? Sorry if this was meant to be a joke and I didn't get it...
Actually it is a fairly normal procedure for collector to trim the gray part of Naso tangs dorsal fin, so the fish looks better.i hope you were being facetious...
no one does that to the fish so it could look cool or different... they're just deformed from birth sometimes... during the collection times, they don't pick them out from the ocean, they just bag 'em up and ship them out...
if you talk to breeders; especially koi/goldfish/carp type breeders... they go thru a process of culling the fry... some of the fry come out deformed or just don't have good coloration, so they just kill them all before they weaken the gene pool down the road... it's a perfectly normal process... but since tangs get shipped... by the time they are shipped, it's too expensive to cull out the deformed ones... since they have allready been shipped... go to the distributors... you will see a bunch of deformed fish...

Sometimes unhealthy Yellow tangs, will lose some of their dorsal fin, but usually retain that front part, the 'horn'. Here's an example;
However if you notice, the rest of NVTE's yellow tang appears to be normal.
And even if it wasn't intentional, the LFS should really hold themselves to a professional level, and not try to make money off unsuspecting customers, by selling a "yellow horned tang," which doesn't exsist
He is not multilated Yellow Tang. He was unwanted fish in the batch, many other thought he is deformed fish, but I can see the beauty in him, so I bought him. Just like those deformed Asian Arowana (freshwater fish)

Heart shaped blood parrots..? There are horrible things people do to fishes and if this fish became in demand people are willing to cut it up
