Maybe it was bacterial do you not have a place to send fish for a autopsy

A couple of thoughts:
Koji stated that he uses fresh water when he brings them in. I wonder if with these little guys that is the way to go? I would have done the same thing everyone else did in this situation putting them in a preconditioned QT, however maybe the route in the future for these deep water specimen it would be better to to do a 100% water change prior to then arriving and then add a large amount of live rock with plenty if critters for then to feed on. Also due to the feeding regiment there will always be a lot of floating and settled food around the tank. Maybe 25/50% water changes ever other day? Also, thinking the addition of an air stone might help?
Breathing heavy tells me that there was something else bothering the fish. I would put my money on the fact that they had not eaten properly since they were caught and just never gained enough strength for the long trip.
Another thing that I have found very helpful in qt is to drop the salinity to around 1.022. Typical reef settings are very stressful to new fish as there is a lot of other elements we put in the tank for the corals to keep them beautiful.
Oodnium would be my last though as the fish sounds clear and only breathing heavy. If it were velvet the Boylei would be gone already.

I too think there was an underlying factor...
I did a 50% change before she went in, no air stone but lots of surface agitation...salinity is 21-22 in all tanks...
the qt tank has been empty of fish since mid-aug but I still fed a couple times a week and did 40 gal wc every week or 2...
Mine just never had the zest to eat more than a tiny bite...Karen said one of hers seemed to not recognize food as food and I saw this too...mine was initially fascinated by the pods but didn't eat any, at least that I saw...
I offered many foods and of many different sizes...
Next time there may be a rest stop for the fish before they get shipped out to their final destinations- this could be huge, imo...

I know hindsight is 20/20 but I was wondering if it would have made sense or a possibility to ship a chunk of the rock of where the pep resides on the next shipment of them?
it's too late for Rufus to do that but it had me thinking it might contain some of the bugs, sponges or algea it uses to sustain itself. Maybe foods we're not aware of? Just typing out loud ...

true...freezing a fish will disrupt cell structure and it will kill bacteria and parasites....
a gross examination of internal organs could be done though it would be suboptimal...you could see an intestinal obstruction or blockage, internal hemorrhage or gall stones.....
but I guess this wouldn't help...

Ted, Sorry I missed the part about the body to study. I was posting on my lunch break and trying to be quick.
If I have a fish that dies I do necropsy but I really am not the best person for the job (not the most skilled at this). Many states have a fish vet and most public aquariums have a disease specialist. I've gotten help from them in the past and they usually are quite helpful. If you want I can try to find out if there is someone in your area. Hopefully you won't need them in the future but it's always a good resource if needed.
Just to add to my previous post, I have been using antibiotics and I did three formalin dips for external parasites just as a precaution even thought the fish didn't show any signs of parasites. I do that with all fish coming into my place. I am very wary of using a lower salinity to control external parasite because it does not eradicate the parasites and they can come back with a vengeance.
The peppermint I have that is eating is now tasting everything I offer. I have seen other new angelfish that don't seem to recognize the foods offered in the beginning so I don't think it's a peppermint only thing. For the one I have here I think he had to feel comfortable and settle in before he wanted to eat.
