Feather duster leaving tube..crown intact w/ body

Has anyone had a intact (worm w/ attached crown) worm leave its tube? I wouldn't think a healthy feather duster would leave its tube and expose its tender body to possible predators. My feather duster will still retact its body w/ crown back into the tube, but then it comes back out again exposing its body. I think its dying...its never been quick about pulling it crown back into its tube....more like slow motion. The only thing new I have added to the tank is 2 peppermint shrimp. Would small shrimp attack a large feather duster?
 
Ours just did that last night, at the same time as seeing that a powerhead fell and the water would have been hitting him directly. About 1" of it's body is exposed, but the crown is still "bloomed."

If anybody has information on Baussie's or this one.....can you please help. I just searched the forum and saw no mention of this topic.
 
I've never had a feather duster, but once my cocoworm crawled out of it's tube, it died shortly after.

I read some where that when stressed to feathers would fall off, or they some times do leave there tubes only to move somewhere else to start a new one...
 
Ours climbed out of its tube one night and died. The body was very thin. No signs of distress before this happened.
