Fungus & Fin Rot? Help! (pic)

My Pajama Cardinal has this fuzzy looking stuff on him and now his tail looks like it is shredding. He still eats fine and socializes normally. Is this Fungus & Fin Rot? He is the only one in the tank that has it. How do I proceed and what do I use for treatment?
