I do not see any ICH on the fish, but the picture does not show enough detail to know. If you referring to the round spot below his eye, that is not ich, but I do not know what it is.

Definitely not Ich. When you have Ich its looks like your fish has been salted. It might be some type of parasite or worm. Google it!

I just read on some other forum that it is head and lateral line disease that starts at the head and slowly progresses. I guess it happens when they don't have enough vegatation in there diet. And the way to correct it is to supplement with vitamin c and organic romaine lettuce. Hope this helps

I would repost in the fish disease for to be sure.
IMO it looks like a bruise or lesion from rubbing up against some reef rock.
