Help! SailFin Tang with dark spots!

Good afternoon,

I have had my tank for 3+ years and never had any issues with my Sailfin Tang, until now.

He has some dark spots forming on his face and around mouth and also like a mucus forming on the top of his head. I have pictures attached.

He is also changing colors very fast to a really dark grey to a slight grey. Hes also rubbing himself.

Any help would be much appreciated! im really worried about him

Black Ich mabye? If not possibly a bacterial infection of some sort. Praziopro or formalin dips can treat black ich.

Google a picture of black ich to confirm if you can.

Thanks, I did look and I cant find very good pictures of it. Does black Ich form around the mouth??

I imagine it can form anywhere it wants. It's a worm. I'm not sure by your pictures that is what it is, just a thought.
