Yes LTA = Long Tentacle Anemone. The scientific name is Macrodactyla Doreensis if you wanna look them up on Google. I don't really know what to say for help though, it's in pretty bad shape. I guess make sure your water has nitrates under 20ppm, phosphates under .03, no ammonia or nitrites... Salinity should be 1.023-1.025, alkalinity should be 10-12. These are all numbers that will make things worse if they do not match up.
Also, being new to the hobby, you need to know that since all these parameters have to be SO specific, it is usually a bad idea to put an anemone in a tank that is under a year old. And it is always a bad idea to get an anemone until you know what kind it is, so that you can tell if it is healthy, and what environment it will want. For instance, the LTA is a sand anemone, and (when healthy) will want to be buried in at least 3 inches of sand, with low flow and VERY bright lights.
