I can't see see any signs of flukes or anything else to warrant prazi treatemnt from the picture. Any symptoms other than hanging on top :twitching or scraping, heavy breathing?
Prazi is pretty safe at recommended doses;some always use it as a preventative treatment for flukes and other worm infestations.
I prefer to avoid medications unless I can determine a need for them as they can suppress appetite.
Can you get a look into it's eyes? Typically , flukes can be seen there as small cloudy patches. They are hard to see on the fish; since they are translucent and colorless . They drop off in a fresh water dip or formalin bath and can been seen in the water or bottom of the vessel used for dipping.
The fish looks healthy in the picture but it's always hard to tell much that way.
It's possible, it has some gill tissue damage from an earlier ich attack but there is no way to know that while it's alive.

Beserk could have been from the irritation of flukes reacting to the freshwater removing about coming off Fish usually do fine for at least 5 minutes in afreshwater dip in dechlorianted water. When flukes jump off they leave bare patches where they have eaten tissue and bleeding can be an issue. I imagine it doesn't feel good when that happens. Sorry , for your troubles.

What are you feeding them? Some mysis or brine might encourage them. Are they breathjing well? Any scraping or twitching? Were they eating at the lfs? Do you have resting /hiding places in the tank (pvc )?
would you QT fish from another reefer's tank?... i am receiving 2 clown fish from a 5 yr old reef tank, i've seen them myself and they both appear to be healthy...
tom, thanks for your help with him, but unfortunately he didnt make it
he was doing awful last night, getting stuck in the air bubble current and tossed accross the tank. completely vertical and struggling even worse... i knew it was only a matter of hours.
so i tried one last effort to get him a freshwater dip (to maybe dislodge anything stuck in his gills if that may have been the problem). well it didnt go well, he sank to bottom and ever 10 seconds would bug out. then after 3 min, he floated on top, i thought for sure he was dead, so i put him back in the QT, he floated for 10 seconds, then actually started breathing again. and went back to his normal awful state.
when i woke up he was dead. here is a pic, you can see something wrongs with his skin and gills. it was an awful 10 days for him. can you see what was wrong with him, so i can make sure his partner doesnt get it?
do you think he would have died in the care of an experienced reefer and my inexperience is why he died?
It's a terrible thing to watch. Sorry for the loss. It's worse when a whole tank gets infested and you see them suffer one after the other. Quarantine and prevetative treatments help prevent those kinds of plagues.
I can't tell. I'd guess, brooklynella or flukes ,perhaps a secondary bacterial infection. Someone with a microscope and good identification skills might.
I've taken in rescue fish from friend's and a couple of local shops. Often, there is nothing we can do to save them. Sometimes we get a nice healthy fish when it all looked bad at the start,though.
Damage to the gills and a loss of oxygen in their system can damage them too severely:internal organs shut down; the brain can't function ;a variety of infections take over; they often loose they're ability to swim and remain horizontal.
Some diseases are difficult or impossible to treat. Hobbyists generally, deal with the most common and most treatable afflictions.
In retrospect, I doubt this fish could have been saved with the treatments available to us. Sounded like it had some severe damage when you got it. If it was flukes ,the color variations could have been from blood loss when they moved off leaving open wounds internally and externally. I think you did all you could reasonably do.
