quarantine tanks

I have a question about quarantine tanks. This is going to have to be theoretical at this point because I don't have a tank at the moment (I plan on getting one in a couple months, but I'm moving soon, so not before then), and I couldn't search previous threads so please bear with me.

When you set up a quarantine tank do you just generally keep the filter material (ie floss or sponge or whatever you're using) in the tank, and when/if a fish gets sick then fill up the tank and plop fish in--obviously watching NH3, NO2, NO3 levels etc very closely?

When dosing these animals with formalin or medication of choice do you dose, then water change, dose, water change etc. For example it seems that the treatment with formalin seems to be to add it to the tank every couple days. I'm assuming that the formalin isn't building up in the tank--because my understanding is that generally charcoal isn't used in these tanks either due to removal of most medications with the addition of charcoal. How long is formalin around if there is a charcoal filter...is it feasible to use a charcoal filter to help remove the traces of the previous medication before dosing again? Does any of this make sense?

I think I understand the freshwater dips, etc--those are for a specified period of time in that particular treatment, but the management of the quarantine tank and some of it's particulars still confound me.
 
It doesn't really matter if you have the filter ready to run in an instance since you have to let the water in the QT to set for at least one day before you put your fish in (that is unless if you will water from the display tank). If you are going to use water from the display tank, then yes, it would be a wise idea to have everyting to go in a moment.

That's a great article, thanks. I'll have to keep it handy. I feel really stupid. I'm a veterinarian, but I have a ton to learn about fish health.
