100% agree with tripod1404, that doesn't look like any Ick I have ever seen.
Time to play VET, and very good learning as well....just start by getting him into a separate place so you can help him...
 
If it's not sand, then I'd bet on flukes. You can try PraziPro like ppl mentioned above. If it turns out to be ich, you have very straightforward answers: Treat them with copper or hypo or DONT treat them and live with it, if your fish is healthy, they probably won't die from ich.

By the way, you better change your mindset if you think of killing a fish cause it's sick. They're living animals that you've chosen to take care of, so do it!

You can treat with PraziPro in your DT. Just follow directions EXACTLY.
Is he scratching up against the rocks or the sand bed?
 
It's not flukes, not lympo and doesn't really look like ich to me. I'm going with grains of sand :lol:, particularly if they disappear during the day and vary as stated in the first post. Further, if your other fish are symptom free, that counter indicates ich as well. I do think that newbies tend to overreact to ich. Feed your fish well, keep the water quality high and many fish can become resistant. Please do not euthanize the fish, it's unnecessary.
 
I am betting against ick and it really does not look like flukes. I would try a couple of rounds of Prazipro. It is safe and may take care of the problem.
 
You know I had a green warpaint goby that would constantly crawl around my rockwork. He constantly had wounds on his body. If I hadn't known that, I would have thought he had ich also.

Point being, since its a goby, it could just be healing wounds from crawling around the rockwork. Considering none of your other fish show signs of ich.
