Sick bubble tip

I've had this GBT for about 3 months. It adapted to my 20 gallon tank very well. My clown started to host it within a couple of days and the two of them were very happy. About a week ago, it started to move and ended up behind the rocks. It had closed up except for a few tentacles sticking out. I fed it a small piece of silverside just to see if it would eat it and it did. I turned the rock around to see if it would open back up but so far it has only opened a little. My tank is 2 years old..Water parameters are:

Sal..1.025
temp..78F
nitrate..0
phosphate...002
cal..450
alk..10.2

None of these parameters have changed within the last three months. I do a 20% water change weekly. This is a before and after pic...

Two things, Clowns don't host the nem, the nem hosts the clowns.

You don't say what you have for lighting. If T5 and your bulbs are more than 7 months old I'd look into getting new bulbs.
