swivel bracket question

I have had a problem recently with one of my streams that this bracket keeps popping out of the stream . Do these usually need replacing as a normal wear item? or any suggestions to keep it from doing so?

The only way I can think of for this to happen is that the arm was not slid in all the way, sometimes I see people popping them in from up top or they slide them in crooked so only 2-4 tabs are actually inserted. I would double check that all tabs are engaging. The best fix I have found for a loose bracket is to heat it over a stove until the plastic is soft and slap it on a cutting board so the tabs broaden and flatten and then it will really lock in.
