Weird worm. Let me know what you think this is!?

Found this in my tank strange pale almost clear round worm, has three black rings around his neck and obvious black eyes. Seemed to wiggle his butt to try and swim away when I got him with my tongs. Looks like he has antennas on his head. Help!
