Get well soon Minh! I had back surgery in June and am going to be having another probably in Feb. :( I feel your pain.
 
The outer layers of anemone is so thin they. An tear easy. The foot is much tougher so they don't tear as easy. When anemone injured the wound just contracted and heal shortly if the anemone is healthy. You see this when carpets loose tentacles. All you see is a shorter tentacle and the tip color is different. As they heal the feta les elongate and the composition of the skin there com back to normal tin a few days, color of that tentacle normalized.
Stressed and unhealthy anemones are unable to heal anywhere near as quick or at all. The result is the wound get bigger and mesothelial filaments spill out as in the picture above.
These wounds are not likely there before but there now. You did not see it yesterday because it was likely not there, not because you and the LFS missed it.
Your anemone is not at all the same health now as he was when he was at the distributer. He is a lot worst.
Continue treatment and hope for the best. Change 100% of the water daily instead of 50 or 75 percent. As long as your water is good and matched salinity and close temp 100% water change then add new med is much better by far

Should i up the dosage to 500?
She has not pancaked after i started treatment.
When i took her out the dt i was very careful i used a plastic bag and disposable glows so im not sure why she would get tears like that.almost as if she has been punctured with a needle.
 
I would use 250 mg per 10 gal. That would be enough to treat. At this dose even if the anemone disenerate it does not smell meaning no bacterial growth. Anybody who treated at this dose that fail will notice that treated dead anemone does not smell
 
So if it dies it wount be because of bakterial infection?
So with propper lights and water quality it should recover?

Wounds have shrink-ed i think.
Shop still closed today but foot is looking better and has attached to the bowl
 
She has completely attached to the bowl and im just gona let her be there fer couple of days.rather not try to move her on to an eggcrate.

Well cipro is over and she is back into dt.i was not seeing any improvements last days but after she went into dt she is significantly better.
I also added the 2 T5's permanently so total of 170 LED wats and 48W T5

This time i made the bottom glass free of sand where is placed her and she seems settled.

Now just to start scouting out a bigger tank..

10 cm is deep enough. About 4 inches. The way the picture is crop it does not seem to be the case. If it is 10 cm your anemone will like it there and will not move around much proved that the light is good, flow is light and water quality is great. He will be fine. Best of luck
