The bad news is that my new Tomini Tang just died. The good news is, he was in the quarantine tank. It gives me a chance to urge everyone to quarantine their new fish. I don't know what killed him, but I'm sure glad not to have exposed my Display tank to it.
 
I have never quarantined a fish in all my years of fishkeeping, but I've learned my lesson the hard way. Added new fish to my koi pond about 2 weeks ago and now I'm treating all my fish for flukes and parasites - at a cost of $150. Three have already died - hoping at least half of the rest will make it.

Lesson learned.....

Ctenochaetus are well known for possibly carrying bad stuff.
 
Sorry for the loss. Gald you are using a qt. The last few fish I've taken in were prophylactically subjected to the tank transfer method for crptocaryon irritans ( new tank every 3 days for a total of 4 transfers) in lieu of the copper treatment I've used in the past. Not as hard as a thought it would be to do and the fish surely is better off without the copper. Not much ammonia build up in a 3 day stay either. I watch for flukes during this period with prazi pro at the ready and/or do a formalin bath for some specimens.

I get the theory of the transfer method and have even considered using this method as well. Is your QT period 12 days or are you leaving it in the 4th tank for two or three weeks after it's introduced?
 
12 days does it for ich and if there is brooklynella or amyloodinium(velvet) it should show up during that period. If it does then a longer period of qt with proper medication( formalin or copper ,respectively) can be done. It wouldn't hurt to move the fish to extended qt preferably in a larger tank, with say prazi pro treatment for those where flukes are common like wrasses can be since flukes sometimes take a few weeks to be noticeable unless you look very closely.

I've only taken in a few small fish since I stared this method one or two ata time ( a small juvenile sailfin, a small yellow tang and a pair of juvenille alardi clowns ) and use just 2 ten gallon tanks which are easy to empty ,dry ,refill and rotate. An ml of Prime ammonia detoxifier in each and light feeding works fine for ammonia control. Med free and less stressful for me and I think the fish as well.
