I've dealt with ich and velvet in the past, never fun.
Catch the fish and treat then in a QT tank. Easiest method for treatment is QuickCure by AP Pro. Simple to follow instructions. I've used this for both ich and velvet with 100% success.

I had pretty decent results with heavy feeding with foods soaked in selcon and garlic as well as treating with Herbtana. Do you know if it came from a fish you purchased or if its something in your tank that has caused it. Temp fluctuations? Stray Voltage? Ammonia? stuff like that....
